New Patient Information Form
Welcome to Allard Station Dental. Filling out our new-patient form before you arrive is the single best way to get the most out of your first visit — it gives our team a head start on understanding your medical history, current medications, and your goals for your smile.
We've kept it short, plain-English, and easy to complete on a phone, tablet, or computer. The whole thing takes most patients under five minutes.
- Basic contact and emergency contact details
- Medical history and current medications
- Insurance information for direct billing
- Your goals and any concerns about past dental care
Dental Plan Form
If you have dental insurance, this form lets us bill your provider directly so you only pay your portion at the appointment — no upfront cost, no waiting weeks for reimbursement, no paperwork to chase down later.
Just have your insurance card or policy number handy. We accept all major Canadian dental insurance plans and most government programs.
- Direct insurance billing — pay only your portion
- All major Canadian carriers and government plans accepted
- Quick verification before your appointment
- We can also help check your remaining coverage for the year
Dental Treatment Consent Form
Before any treatment beyond a routine cleaning, we walk you through the procedure, the alternatives, the risks, and the costs — and we ask you to acknowledge what you've understood with a treatment consent form. It's both a regulatory requirement and our way of making sure you feel completely informed.
Read it at your own pace, ask us anything, and only sign when you're confident. We never rush this conversation.
- Clear explanation of every recommended procedure
- Plain-English summary of risks and alternatives
- Itemized cost breakdown — no surprises
- Time to ask questions and discuss with family
Dental & Medical Health Form
Your overall health and your oral health are deeply connected — a heart condition, a recent surgery, or a new medication can all change the safest way to treat your teeth. Our medical health form gives our team the full picture so we can plan treatment that's safe and effective for you specifically.
Anything you share is kept strictly confidential and is only seen by your treating clinicians. If anything changes between visits, just let us know.
- Comprehensive medical history (kept strictly confidential)
- Current medications and known allergies
- Past surgeries, dental and otherwise
- Pregnancy, blood-thinner, or special-precaution flags
